First Visit

(Initial Evaluation)
Your initial appointment will consist of any necessary x-rays and a comprehensive review of your health history, periodontal charting, and an evaluation by the Dentist. The Dentist and your hygienist will consult with regards to your hygiene needs and schedule you for your initial cleaning or alternative treatment that is deemed appropriate for your specific case as well as determining the proper length of time needed.

A comprehensive treatment plan will also be developed for you that will address any other dental needs that you may have to be presented at your initial cleaning appointment. Our treatment coordinators will work with you to schedule all necessary appointments as well as answer any questions that you may have with regards to your dental insurance coverage and / or any portion you may have.

(Emergency Treatment)
Your initial appointment will consist of any necessary x-rays and a consultation explaining your diagnosis and treatment options. Depending on the particular dental need that you may have, treatment may be performed the same day as the consultation. However, a complex situation, medical history or treatment plan will require a more through evaluation and a second appointment to provide treatment on another day or referral to a specialist.

(Information requested)

Please assist us by informing us of the following information at the time of scheduling your appointment:

• Any X-rays that you or another Dentist office may have
• A list of your current medical prescriptions and any known need of a pre-med (antibiotic) prior to any dental procedure
• Any medical situations that we may need to be aware of such as: pregnancy, hip or joint replacement, history of - heart murmur, irregular heart beat, high blood pressure, stroke, rheumatic fever, or recent hospitalizations or any other major medical concerns
• Dental insurance information (if applicable): name of insurance, phone number, plan #, subscriber’s social security number, & birth date (insurance requires this to verify and release information to us)

Children

We begin receiving children as patients as early as 4 years of age.
A parent or legal guardian must accompany all patients under the age of 18 for their first appointment and all appointments for children under the age of 16.
All children under the age of 16 must be accompanied by a parent or legal guardian for all appointments and may not be left unsupervised (whether it is the child’s or a parent’s appointment).
